Sometimes, in bed with a woman, she laughs.  I expect a little tickly giggling now and then, but laughing? What am I doing wrong?

You are close to understanding this. You already know there are different kinds of laughing, and you accept giggling in response to being tickled. People don't always laugh at things that are funny, sometimes they laugh when they are just really happy. 

I suspect that you are not doing anything wrong, but a lot of things right.  I know that when I give Sweetie a blowjob, he sometimes gives this sort of helpless laugh and sigh, "Hee hee heeeeeaaaaah!"  I love it!  It means he's feeling so good he can't talk. 

I myself will often let out a low, contented chuckle when I am feeling good.

So, unless you are getting derisive laughter, take the laugh as a compliment.  Or do you have some sort of strange tattoo I ought to know about?
